Access rules do apply. If you are interested in this access, you will be asked to fill out a proxy consent form. Once the form is signed your care team will change your record to reflect your request.

Understanding your Child or Teen’s Record

A parent may obtain full proxy access for their child if the child is under 12 years of age. Due to privacy laws proxy access for teens, considered patients age 12 through 17 years, may be limited based on the preference of the teen. Teens and adults granting access to others must sign the form.

Physical Medicine and Rehabilitation

Physical Medicine and Rehabilitation– also called Physiatry  – is a branch of medicine that specializes in diagnosis, treatment and management of disease primarily using "physical" means (such as physical therapy and medications). Essentially, physiatrists (fizz eye'uh trists) specialize in a wide variety of conservative treatments (nonoperative) for the musculoskeletal system (the muscles and bones) and do not perform surgery.

Endocrinology

Doctors of internal medicine (internists) focus on adult medicine and have had special study and training on the prevention, detection and treatment of adult diseases.

Kadlec Clinic offers comprehensive, caring family medicine at four neighborhood-based clinics, some with extended hours, so that medical care is available where you need it, whether that is close to home or close to work. The specialty of family medicine treats people of all ages, from newborns to seniors. This allows one physician to serve as the primary care physician for the entire family and coordinate all of your family’s care.

Kadlec Clinic’s family medicine practitioners are highly trained and provide a wide range of services including preventive medicine, care of ongoing chronic illnesses and treatment of acute problems. And, you can go with confidence because we choose the kind of physicians that we ourselves would like to have care for our families. Each provider’s credentials and training have been extensively reviewed.

Our primary care clinics offer:

Family Medicine -- caring for patients of all ages

Family Medicine Residency -- caring for patients of all ages, with extended appointment times supervised by teaching physicians

Pediatrics -- specializing in children from infants to age 18
