To insure accurate billing and timely claim submission, please provide the following information when you register:
Please Provide :
- Current insurance identification card
- Labor & Industries claim number and date of injury
- Medical coupon
- Retirement and/or disability date
- Doctor's orders and surgical consent form
- Insurance approval letter or authorization letter
- Photo Identification
A yearly authorization card for the billing of insurance is available at your request. Also, information is available on advanced directives, patient rights and responsibilities, Medicare and Champus rights.
